A dental hygienist is one of the professions that is highly paid in the united states with a gross salary of 62,430 per year, this salary is the mean wage rate for the profession provided by the united state bureau of statistics, every other employee is required to pay taxes and gross salary minus the taxes give you net salary, the taxes we consider in this profession include social security and Medicare taxes, state taxes and federal taxes.

In our calculation of the net salary we will consider the federal taxes, state taxes and Medicare and social security taxes, we assume that the employee works in Arizona State and therefore his state tax will be with reference to the Arizona tax rate. For this reason this paper will first consider the calculation of individuals taxes using the gross salary and therefore we must consider the gross salary first and then search for appropriate sources of the tax rates that are required in the calculation of the net salary.

Gross salary:

The mean gross salary of a dental hygienist is 62,430 per year, this according to the united state bureau of statistics because taxes are paid every month we will have to determine the mean salary per month and this will be obtained by dividing the annual mean salary by the number of months per year which is 12,

Monthly gross salary = annual gross salary / 12 month

= 62,430/12

= 5202.5

Medicare and social security taxes:

Given the monthly gross salary we are in a position to calculate the Medicare and social security taxes, this tax level is obtained by multiplying the gross profit by the tax rate which gives us the tax level.

Tax = gross salary X tax rate

The social security tax is 6.2% and the Medicare taxes is 1.45%  we therefore consider each tax separately and this will be as follows:

Social security tax:

6.2% X 5202.5 = 322.555

Medicare taxes:

1.45% X 5202.5 = 75.43625

State tax:

State tax is also deducted from the gross salary in order to get the net salary that a dental hygienist should receive, the state tax for the Arizona state lies between 2.59% and 4.54%, for this reason therefore we get the mean rate of the Arizona rates, these rates were retrieved the mean rate will be achieved by adding up the two rates and then dividing the sum by two, this will be as follows:

2.59% + 4.54% = 7.13%

7.13% /2 = 3.565%

For this reason therefore we will consider the state tax as 3.565% which the mean rate of tax in Arizona. Given the rate of tax as 3.565% we can find the tax level of a dental hygienist in Arizona and this will be given by multiplying the gross monthly salary by the tax rate, therefore our tax level will be achieved as follows:

5202.5 X 3.565% = 185.4691

Therefore our state tax per month is 185.4691.

5202.5 X 10% = 520.25

Therefore our federal tax level is equal to 520.25.

Net salary:

Having determined the tax that is included in calculating the net salary we can now determine the net salary, the net salary is achieved by subtracting the all taxes from the gross salary, in our case therefore the following will be our calculations:

Net salary = gross salary – (Medicare tax + social security tax + federal taxes + state tax)

This can therefore be simplified in the table below:

Assumptions of this calculation are that the dental hygienist salary is 62,430 per annum and therefore to get the salary per month is given by dividing this amount by 12 months. The other assumption made during calculation is that the person in question who is a dental hygienist is a single person and not married, the other assumption made is that the taxes rates given are monthly tax rates and finally the assumptions is that only federal taxes, state taxes and Medicare and social security taxes are relevant in the calculation of net salary, this assumption also states that there are no tax refunds or compensations such as transport allowances and house allowances provided to the dentists.

Physical properties:

Zirconium is a grayish white metal, shiny, very resistant to corrosion and is lighter than steel. When finely divided can burn spontaneously in air (prior to nitrogen react with oxygen), especially at high temperatures (around 500 ° C). It is a metal resistant to acids, but can be dissolved with hydrofluoric acid.

The zirconium melts near 2715 ° C. It is estimated that its boiling point to 4377 ° C, but some observations suggest that it is near the 8600 ° C.

Collection:

The zirconium is not found in nature as free metal, but part of many minerals. The main source of zirconium is obtained from the mineral zircon, found in deposits in Australia, Brazil, India, Russia and the United States. It is also found in other minerals as Baddeleyite.

History of zirconium:

In 1975 the British physicist Ron Garvie published in renowned business journal Nature his striking work under the title ‘Zirconia: Ceramic Stell’ (zirconia, ceramic steel “?). His research on the possibility of stabilizing the tetragonal structure of zirconium dioxide by adding about 5.5% yttrium oxide material helped to achieve the exceptional mechanical properties and high biological stability.

The resistance to bending, the shear strength and modulus E reached values of steel, therefore, the ceramic Garvie also called ‘ceramic steel’.

A few years ago entered the field of dentistry for the performance of ceramic copings for crowns.

The zirconia is considered one of the best ceramic products on the market for dental reconstructions, from the 90s, it is increasingly used in the field of dental technology. It is estimated that between 15,000 and 20,000 zirconia structures are made each day worldwide.

Numerous tests have confirmed that a successful zirconia bridge is sufficiently strong even after 50 years. Studies have shown that zirconia does not produce any kind of contact allergy to the soft tissues in humans.

Zirconium is used in dentistry is zirconium oxide (zirconium dioxide), which when stabilized with yttrium generates an extremely hard ceramic material.

Possibilities of the zirconium oxide to the clinician:

The zirconium for their properties, hardness and biocompatibility, gives you the option of working in dental surgeries nonmetallic tools (cutters zirconium) to avoid any metal contamination that may be caused.

Furthermore, the rotary instruments made of zirconium oxide has advantages in dentoalveolar surgery in terms of temperature control and in economic terms can be reused a number of times and show no alterations or wear on its cutting blades.

The biggest advantage given to zirconium is that the dentist has the option of placing an implant of zirconium oxide rather than a metal one. The zirconium oxide implants are more stable from a mechanical point of view, besides that have opened the possibility of a new generation of color dental implants.

The most common application of zirconia is to develop dental veneers, crowns, inlays, though today prosthesis can be made of 14 pieces.

Due to its high biocompatibility, strength and esthetics it is today the best option to perform oral rehabilitations.

Having a sharp hygiene team is a beautiful thing. But assuming everything is being taken care of is a big mistake. Be sure you know that a complete perio exam is taking place at every hygiene visit AND the data is being recorded. This is a huge area of liability for you and it’s your hygienists responsibility to collect and analyze this information. One way to find out where you stand is to do an audit of your charts. Randomly pull 20 charts of adults seen in the last 6-12 months. How many have a complete perio exam recorded in the last year? Chances are very good that if the percentage is low, so is the amount of perio treatment.

#2 Starting Perio Treatment Too Late

My interpretation of the AAP’s Perio Classification system is that Beginning (slight) periodontal disease is 4mm pockets with bleeding and slight bone loss and 1-2mm CAL. This is a huge distinction for many dental teams. Often, these are the “difficult prophies” or the patients that have 3 month prophy intervals. I see it time and time again- teams waiting until pockets are 5-6mm deep before beginning treatment while valuable the patient loses valuable bone that could have been saved.

#3 Too Little Time for Hygiene Visits

I’ve mentioned this before and I’ll say it again. Every time I have ever coached a team to increase their hygiene time, their production has increased as a result. Now that wasn’t they only thing they did. They put in place systems to deliver a higher level of service and enroll more treatment. Critical steps that drive production are left out of the hygiene exam when time is short. When you choose to allow 60 minutes for adult recare and perio maintenance, it’s important to outline exactly what will take place during that time. If you are going from 40min hygiene visits to 60min, what are you going to add?

Wait- Here are a few action steps to help you get started NOW:

• Do a chart audit-look for current perio charts
• Communicate to your hygiene team that you’d like them to do a complete perio exam on every adult patient
• Give your hygiene team the time they need to complete a comprehensive hygiene exam
• Be proactive and get the information you need to really believe that starting perio therapy early is the right thing to do

Think back to the last time you attended a really great CE course. You came back with some great ideas and wanted to get started right away. Then you pull out your notes and realize there are 10 or more new products, services or ideas you want to put into place NOW. I know the feeling. I’ve been there myself. And I know that the next thing you feel is overwhelm. Where do you start? How am I going to fit all this into my 50 or 60 minute appointment time? Are my patients going to think I’m just trying to sell them the next best toothbrush?

Then you get that feeling that stops you in your tracks…I just can’t do it all! The good news is, you don’t have to! Here are a few tips to help you systematically incorporate all that you’ve learned, over time.

Sit down with your notes and make a list of all the new products, techniques, services you want to begin offering to your patients in the next 12 months. Then rank them with #1 being something that you must do NOW. This would be something critical like doing an oral cancer screening on every patient. Somewhere lower on your list might be completing a Caries Risk Assessment on every patient. Maybe your already doing a complete perio exam, oral cancer screening and Caries Risk is high on your list of To-Dos. This is going to vary for everyone

Then create a timeline for when it will be implemented. Maybe you start doing visual & tactile oral cancer screenings on Monday. Then you request a lunch and learn on the Velscope or ViziLite on Wednesday knowing that it will be next month before you make your decision on which testing system to use. Get that system implemented and running well and then focus on the next item. Maybe the Caries Risk and Treatment System will be 6 months out. That’s ok, just have it in your long-range plan.

Every team is going to have different priorities but there are some standards of care that must be addressed quickly. These are musts in EVERY hygiene exam:

• Oral Cancer Screening
• Complete Periodontal Exam with charting
• Medical Review

These are important but may be on the 3-6 month To Do list:

• Cosmetic evaluation with shade guide
• Caries Risk Assessment Form and Treatment System
• Implement a new Homecare System

We all want to do everything tomorrow but the reality is that if we don’t prioritize, we’ll get overwhelmed and not implement any of it. Now take a breath and make your list.
